Reading the Work

Helping clinical teams see what stood between patients and safe discharge

Harborwell Health needed a clearer way to coordinate patient discharge planning across clinical teams, family caregivers, transportation partners, and community providers. The work was highly collaborative, but the supporting information was scattered across notes, phone calls, queue exports, and department-specific systems.

Wolfcrest & Co. worked with care managers and operations leaders to define the common blockers that slowed discharge readiness. The resulting workspace organized tasks by patient, dependency, owner, and urgency, making it easier to see whether a patient was waiting on medication reconciliation, equipment delivery, transportation, follow-up scheduling, or caregiver confirmation.

The experience helped Harborwell teams focus conversations on the next decision rather than the latest status hunt. Leaders gained a better view of system-level bottlenecks, and care teams had a practical tool for coordinating safe transitions without adding another reporting burden.
