Reading the Work

Helping care teams keep referral promises visible after the handoff

VitalBridge worked with community clinics where referrals often depended on availability, eligibility, transportation, language access, and partner follow-up. Teams cared deeply about the handoff, but they lacked a shared way to see whether patients had actually connected with the next service.

Wolfcrest & Co. designed a referral coordination workspace that tracked status, barriers, partner responses, and follow-up timing across clinics and community organizations. Navigators could see which referrals needed attention and which patients required additional support before an appointment was missed.

The result helped VitalBridge make referral continuity part of daily operations. Clinics gained a clearer view of patient progress, partners received better context, and navigators could focus on the handoffs most at risk.
